Implants explained by UK trained implantologist Dr Kiran Haseeb.

In short

What are dental implants?

A dental implant is a small titanium fixture placed into the jawbone to replace the root of a missing tooth. Over three to four months the bone grows onto its surface, a process called osseointegration, which locks it permanently in place. An abutment and a crown are then fitted on top and the tooth is complete.
Unlike a dental bridge, an implant does not require the healthy teeth on either side to be cut down, and unlike a denture it is fixed, so it does not move when you eat or speak. It is the only option that replaces the root as well as the visible tooth, which is why the bone underneath stops shrinking once an implant is placed.
Implant dentistry sits between surgery and prosthodontics, so who does the work matters. Here the assessment, the surgical placement and the final crown are all carried out by Dr Kiran Haseeb, an implantologist with advanced training in implantology, rather than the surgery being done in one place and the crown somewhere else. Gum disease is stabilised first with periodontal treatment care, and bone grafting or a sinus lift is handled as oral and maxillofacial surgery.

The three parts of a dental implant

Part 1

The fixture

The titanium post placed into the bone. This replaces the root, is designed to last for life, and is the part the whole restoration depends on.

Part 2

The abutment

A connector that screws into the fixture once healing is complete and holds the crown at the correct height and angle.

Part 3

The crown

The visible tooth, made in zirconia or E-max and shade matched to the teeth beside it. This is the part that may need replacing after ten to fifteen years.
Options

Types of dental implant treatment.

Which one applies to you depends on how many teeth are missing, where they are in the mouth, and how much bone is left. All three are planned from the same x-ray.
Single tooth dental implant replacing one missing tooth

Lowest cost

Single tooth implant

One implant replaces one tooth, independent of everything around it. This is the option that preserves the neighbouring teeth completely, because nothing is cut down to support it. It is also the easiest to clean, since it is flossed like a natural tooth.
Implant-supported bridge replacing several missing teeth in a row

Mid range

Implant-supported bridge

When three or four teeth are missing in a row, two implants can carry the whole span rather than placing an implant for every tooth. This reduces both the number of surgical sites and the cost, and is the usual answer where a run of back teeth has been lost.
Full arch implant restoration supported by four to six implants

Highest cost

Full arch on four to six implants

A complete upper or lower arch fixed onto four to six implants, often called All-on-4 or All-on-6. For an edentulous patient who has worn a full denture for years, this ends the adhesive, the movement while eating and the covered palate. A fixed temporary bridge is worn throughout.
Cost

What do dental implants cost in Islamabad?

This is the question almost everyone actually arrives with, so it is worth answering properly rather than hiding behind contact us for pricing. A quote is given after the examination and x-ray, never over the phone, because a price offered for a tooth nobody has looked at is a guess. What you receive is a single written total for the finished tooth, not a starting figure that grows at each stage.

What a written quote should include

How many teeth are being replaced

One implant with one crown is the simplest case. Several teeth in a row may need only two implants carrying a bridge, which is usually less expensive than people assume. A full arch is a different scale of treatment entirely.

Whether bone grafting is needed

If the tooth has been missing for years the bone has usually resorbed, and volume has to be rebuilt first. In the upper back jaw this may mean a sinus lift, carried out as oral and maxillofacial surgery. This is the most common reason two quotes for one implant differ.

Which implant system is used

Straumann and BioHorizons cost more than unbranded implants, and are documented, serviceable and replaceable decades later. An unbranded implant may save money now and leave you unable to find a matching component later.

What the crown is made of

Zirconia and E-max cost more than porcelain-fused-to-metal and look considerably better, particularly at the front of the mouth where a metal margin can show as a dark line at the gum.
A word about very low implant quotes. If one clinic quotes far below everyone else, ask three questions: which implant system is being used and can they name the brand, whether the crown is included or priced separately afterwards, and whether grafting has been assessed or will be added later as an extra. A quote is only comparable when it covers the finished tooth. Most of the unpleasant surprises patients describe come from a low headline figure that did not include the crown.

Safety

Are dental implants safe?

Titanium implants have been placed for more than fifty years and are among the most studied procedures in dentistry. Large long-term studies report survival above ninety percent at ten years, which is higher than most bridges achieve. The NHS guidance on dental implants describes the same procedure and the same healing period.
Titanium is biocompatible, which is why it is also used for hip and knee replacements, and true allergy to it is extremely rare. The risks that do exist are surgical and manageable: infection at the site, damage to the nerve in the lower jaw, or the sinus in the upper jaw. All three are avoided by measuring properly on the x-ray before surgery rather than during it, which is why the scan is not optional here.
The one genuine long-term risk is peri-implantitis, gum disease around an implant. It is caused by plaque, behaves like gum disease around a natural tooth, and is the main reason implants are lost years later. It is preventable with gum health care and regular scaling and polishing visits, which is why aftercare is discussed before treatment rather than after it.
Suitability

Are dental implants right for you?

Most adults with a missing tooth are suitable. A few situations need something dealt with first, and a small number are better served by a bridge or a denture. You will be told which of these applies to you after the x-ray, not before it.

Usually straightforward

Needs sorting out first

If an implant is not the right answer for you, you will be told so. A bridge or a well-made denture is sometimes the better clinical and financial decision, and saying that costs the clinic a larger case. It is still the right thing to say.
Step by step

How do dental implants work?

A single implant is normally three to four appointments spread over three to four months. Most of that time is healing rather than chair time, and you are not without a tooth during it.

Stage 1 · Visit 1

Assessment and scan

A full examination and a digital x-ray, an OPG or CBCT scan, to measure bone height and width and to check the position of the sinus and the inferior alveolar nerve. This is where it is decided whether an implant is possible at all, and whether a graft is needed first.

Stage 2 · Visit 1

Planning and consent

The plan is shown to you on screen: where the implant goes, which system is used, how many visits, and the full cost. Nothing is booked until you have agreed to it.

Stage 3 · Visit 2

Placing the implant

The titanium post is placed under local anaesthetic in a sterile field. It takes about 45 to 60 minutes for a single implant. Most patients drive themselves home afterwards.

Stage 4 · 3 to 4 months

Healing and integration

The bone grows onto the implant surface, a process called osseointegration. A temporary tooth can be worn in the meantime so you are never left with a visible gap.

Stage 5 · Visit 3 to 4

Fitting the crown

An impression or scan is taken, the crown is made in the laboratory and shade matched to the teeth beside it, then fitted and adjusted until the bite feels right.

The honest answer

Is getting a dental implant painful?

Placing an implant is usually easier than the extraction that came before it. Bone has no pain receptors of its own, so once the gum is anaesthetised you should feel pressure and movement, not pain.
Longevity

How long do dental implants last?

The implant post is designed to be permanent, and studies report survival above ninety percent at ten years. The crown on top is the wearing part and may need replacing after ten to fifteen years, the same as any crown.
What shortens the life of an implant is almost never the implant itself. It is untreated gum disease around it, smoking, and uncontrolled diabetes. An implant cannot decay, but the gum and bone holding it can be lost exactly as they can around a natural tooth.
Aftercare is simple and not negotiable: brush and clean between the teeth daily, use the interdental brush size you are shown rather than guessing, and attend a hygiene visit every six months so the tissue around the implant is checked and cleaned professionally. Patients who do this keep their implants for decades.

What we place

The implant systems used at this clinic.

The system matters more than most patients are told. A recognised system means the components are documented, replaceable, and any dentist anywhere can service them years later. Unbranded implants are cheaper and can leave you stranded if a part ever needs replacing.

Straumann

Swiss, and among the most researched implant systems in the world, with decades of published long-term survival data behind it. Manufacturer site.

BioHorizons

American, widely used for its surface treatment and a well documented record where bone quality is not ideal. Manufacturer site.
Ask any clinic which system they use. If the answer is vague, or the brand cannot be named, that is worth knowing before you commit to surgery.

Related treatments

Treatments that come before, instead of, or after an implant

If the tooth is still savable, root canal treatment is usually attempted before anything is removed. If it has to come out, read tooth extraction in Islamabad, or wisdom tooth extraction for a third molar. Where an implant is not suitable, a dental bridge or a dental crown restores the gap instead. Gums are assessed by a periodontist first, and grafting or a sinus lift is done as oral and maxillofacial surgery. For the front teeth, patients often combine an implant crown with veneers and smile makeovers, and anyone in pain today should see the emergency dentist in Islamabad. Everything is carried out at the dental clinic in F-10 Markaz.

Before you call

Dental implant questions, answered honestly.

The honest answer is that it depends on four things: how many teeth are missing, whether a bone graft or sinus lift is needed, which implant system is used, and what the crown on top is made of. A single implant with a zirconia crown and no grafting is at the lower end. Full-arch work is a different scale of treatment. You will be quoted after the x-ray and examination, in writing, with the whole figure rather than a per-stage price that grows as you go. Ask any clinic for the figure in writing; the American Dental Association patient guide explains the same components you are paying for.

The implant post itself is designed to be permanent, and large long-term studies report survival above 90 percent at ten years. The crown on top is the part that wears, and may need replacing after ten to fifteen years, the same as any crown. What shortens implant life is untreated gum disease and smoking, not the implant itself.

The placement is done under local anaesthetic and you should feel pressure but not pain. Most patients describe it as easier than the extraction that preceded it. Afterwards there is usually two to three days of soreness, managed with ordinary painkillers. If pain is increasing rather than decreasing after day three, that is worth a phone call.

Sometimes. Immediate placement works when the socket is clean, there is no active infection, and the surrounding bone is intact. If there is an abscess, or the socket walls are damaged, the site is allowed to heal first. That decision is made from the x-ray and what is found at the extraction, not in advance.

This is common when a tooth has been missing for years, because the bone that supported it slowly resorbs. Bone grafting or a sinus lift can rebuild enough volume to place an implant, and adds a healing period before placement. If grafting is not sensible in your case, you will be told that, and a bridge or denture discussed instead.

Well-controlled diabetes is not a barrier, and implants are placed routinely for diabetic patients whose HbA1c is stable. Uncontrolled diabetes is, because healing is impaired. Smoking measurably raises the risk of implant failure, and you will be asked to stop for at least two weeks before and eight weeks after placement. Neither is a reason to be turned away without a conversation.

An implant replaces the root and does not touch the teeth on either side. A bridge is faster and cheaper, but the two neighbouring teeth have to be cut down to carry it, and that is irreversible. If the neighbouring teeth are healthy and untouched, an implant is usually the better long-term decision. If they already need crowns, a bridge can be the sensible choice.
