What do braces and aligners actually do?

Both move teeth by applying light, continuous force so the bone around each root remodels and the tooth travels to a new position. Fixed braces do it with brackets bonded to each tooth and an archwire that the orthodontic plan shapes over time. Clear aligners do it with a series of removable trays, each one a fraction of a millimetre different from the last.

The word that matters in the assessment is malocclusion, how the upper and lower teeth meet. Crowding, spacing, rotations, an overjet, a deep bite, an open bite and a crossbite are different problems and they do not all respond to the same appliance. Simple crowding and spacing are routinely treated by a general dentist with orthodontic training; skeletal discrepancies, impacted canines and surgical cases belong with an orthodontist, and you will be told which category you are in at the assessment rather than after a brace is on.

The three parts of a fixed brace

Part 1

The brackets

Bonded to each tooth in a position decided by where that tooth has to end up. Metal or tooth coloured ceramic. The bracket is the handle the system holds the tooth by.

Part 2

The archwire

The wire that stores the force. It starts thin and flexible to unravel crowding, then becomes heavier and more rectangular as the case moves into detailing and bite correction.

Part 3

The elastics

Small modules holding the wire in place, and larger elastics you wear between the jaws to correct how the bite meets. Elastics are the part where the result depends on you.

Options

Types of orthodontic treatment.

The right appliance depends on what has to move, how far it has to travel and whether the bite itself needs correcting. All three are planned from the same records and x-ray.

Most cases

Metal braces

Stainless steel brackets and wires. The most efficient appliance for rotations, deep bites and anything that needs real control, and the least expensive of the three. Coloured modules are changed at each visit if you want them.

Tooth coloured

Ceramic braces

The same mechanics with tooth coloured brackets, so the appliance is far less visible in photographs. Slightly more fragile and slightly slower for heavy movements, and usually fitted on the upper arch where it shows.

Removable

Clear aligners

A series of clear trays worn twenty two hours a day and changed on schedule. Excellent for mild to moderate crowding and spacing, and for adults who cannot wear a fixed brace at work. Results depend entirely on wear time.

Cost

What do braces cost in Islamabad?

Orthodontic quotes vary because the cases vary, not because the brackets do. A price is given after the examination and x-ray, in writing, for the whole course including adjustments and the first set of retainers, so the figure does not grow each month.

How far the teeth actually have to move

Mild crowding treated in under a year is a different course from a full bite correction over two years. The number of visits is what changes, and that is what the price follows.

Which appliance the case needs

Metal is the most economical, ceramic sits above it, and clear aligners are highest because every tray in the series is manufactured for you.

Whether extractions or extra anchorage are needed

Severe crowding sometimes needs an extraction to create space, and some bites need elastics or temporary anchorage. Those are quoted openly rather than added later.

What happens before the brace goes on

Active gum disease or decay is treated first. That work is separate, and doing it first is what protects the teeth you are about to move.

A word about monthly payment quotes. A figure quoted per month means nothing without the number of months attached to it. Ask for the total for the full course, ask whether retainers are included, and ask what happens to the fee if treatment runs longer than planned. A written total answers all three at once.

Safety

Are braces safe for your teeth?

Orthodontic movement is safe and well documented when the mouth is healthy first and the force is light. The risks are real but manageable, and all of them come back to hygiene and control.

Decalcification, the white marks left around a bracket, is the common one. It is caused by plaque sitting around the bracket, not by the brace, and it is prevented by brushing properly, cleaning between the teeth and keeping hygiene appointments during treatment. Mild root shortening can happen in long cases, which is why the plan is checked on x-ray rather than assumed.

Gum health is monitored at every visit, and any sign of gum inflammation is treated before it can do damage while teeth are moving. If you want an independent explanation of the same risks, the NHS guidance on orthodontics covers them plainly.

Suitability

Are braces or aligners right for you?

Most adults and teenagers with crowding, spacing or a bite that does not meet properly are suitable. A few things have to be dealt with first, and a few cases genuinely belong with a specialist.

Usually straightforward

Crowded, crooked or rotated teeth
Gaps you want closed, including a gap between the front teeth
An overjet, deep bite or crossbite that has never been treated
Teeth that drifted after a tooth was lost years ago
Healthy gums, no active decay, and a willingness to keep appointments

Needs sorting out first

Active gum disease, which is treated and stabilised before any appliance goes on
Untreated decay, which is dealt with as fillings first
Heavy plaque and bleeding gums, where scaling comes before brackets
Skeletal problems or impacted canines, which are referred to an orthodontist rather than started here
Aligner cases where twenty two hours a day of wear is not realistic, which are better served by a fixed brace

If your case needs a specialist you will be told at the assessment and referred, not started and handed on halfway. Equally, if a simple case is being quoted as a two year full-mouth treatment elsewhere, you are welcome to bring that plan in for a second opinion.

Step by step

How orthodontic treatment works, step by step.

Most courses run twelve to twenty four months with an adjustment every four to six weeks. The first and the last appointments are the long ones; the visits in between are short.

Stage 1 · Visit 1

Records and x-ray

Photographs, impressions or a scan, and an x-ray to see the roots, the developing teeth and the jaw relationship. This is what separates crowding that needs space from crowding that will unravel on its own.

Stage 2 · Visit 1

The plan

Which appliance, how long, whether space has to be created, and what the finished bite should look like. You see the plan and the total cost before anything is booked.

Stage 3 · Visit 2

Fitting the appliance

Brackets are bonded and the first light wire is placed, or the first aligners are issued with attachments. It takes about an hour and is not painful, though the teeth feel tender for a few days afterwards.

Stage 4 · Every 4 to 6 weeks

Adjustments

The wire is changed or the next aligners are issued, progress is measured against the plan, and the gums are checked. Missed appointments are the most common reason a case runs longer than quoted.

Stage 5 · Final visit

Debond and retention

The appliance comes off, the teeth are polished, and retainers are fitted the same day. Retention is not optional: teeth move back towards where they started if nothing holds them.

The honest answer

Do braces hurt?

Fitting a brace does not hurt. What most people describe is tenderness for two to four days afterwards, and again for a day after each adjustment, because the bone around the roots is responding.

First week. Teeth feel tender when you bite. Soft food and ordinary painkillers cover it.
Cheeks and lips. Rubbing settles once the tissue toughens. Orthodontic wax is given at the fitting for the first few days.
After adjustments. A day of tenderness, considerably milder than the first week.
Aligners. Pressure for the first day of each new tray, which is the tray working rather than a problem.

Longevity

How long do the results last?

The teeth stay where the retainers keep them. Treatment ends with a fixed wire bonded behind the front teeth, a removable retainer worn at night, or both, decided by how the case started.

Relapse is the risk everyone underestimates. Teeth have a memory, and lower front crowding is the movement most likely to return in the first year if retainers are not worn. Wearing them is the difference between a result that holds for decades and one that drifts.

Retainers are checked at your routine dental visits, and a fixed retainer wire is inspected at every scaling and polishing appointment. If a retainer breaks or stops fitting, it is replaced rather than left, because a few weeks without one is enough for movement to start.

Sequential tray systems with bonded attachments where the case needs grip. Suitable for mild to moderate cases, and honestly ruled out where a fixed appliance would do the job better.

Ask any clinic what happens if you move city mid-treatment. With a documented system another orthodontist can pick the case up; with an unnamed one that is much harder, and it is worth knowing before you commit to two years.

How much do braces cost in Islamabad?

It depends on the appliance and the length of the case. Metal braces are the most economical, ceramic sits above them, and clear aligners are highest because every tray is manufactured for you. How long will I have to wear braces?

Most courses run twelve to twenty four months. Mild crowding can finish sooner, and bite correction with elastics takes longer. The estimate is given from your records at the start, and the honest variable is you: missed adjustment appointments and poor elastic wear extend cases more than anything the appliance does.

Am I too old for braces?

No. Teeth move at any age as long as the bone and gums are healthy, and a large share of orthodontic patients here are adults. What changes with age is not whether teeth move but how carefully gum health has to be managed while they do.

Braces or clear aligners, which is better?

For mild to moderate crowding and spacing in someone who will genuinely wear them twenty two hours a day, aligners are excellent and far less visible. For rotations, deep bites, closing extraction spaces and anything needing real control, fixed braces do the job more predictably. The case decides, not the marketing.

Will teeth need to be removed for braces?

Sometimes, and only when there is genuinely not enough room in the arch. It is decided from the x-ray and the space analysis, never as a routine. Where space can be created by expansion or by reshaping contact points instead, that is done first.

Do braces damage the enamel?

Not by themselves. The white marks people worry about are plaque damage around the brackets, which is prevented by brushing properly, cleaning between the teeth and keeping hygiene appointments during treatment. That is checked at every adjustment visit.

Do I really have to wear retainers?

Yes, and for the long term. Teeth drift back towards their old position, and lower front crowding is the first thing to return. Most patients end with a fixed wire behind the front teeth plus a night-time retainer, which is a small habit compared with repeating the treatment.
