Plaque is the soft film that forms on teeth every day. Left alone it hardens into calculus, also called tartar, which is rough, porous and bonded to the tooth. No toothbrush removes calculus. Scaling is the procedure that does, with an ultrasonic tip and hand instruments, and polishing smooths the surface afterwards so plaque takes longer to stick again.
The reason it matters is the gum, not the appearance. Calculus sitting at the gum line keeps the tissue inflamed, which is gingivitis. Left long enough the inflammation moves deeper, the bone around the tooth is lost, and that is periodontitis, the most common reason adults in Pakistan lose teeth that were never decayed. Calculus begins forming again within days, so the honest answer is that a clean is not a result you keep, it is a reset. What decides how long the mouth stays healthy is what you do at home between appointments.
