What is scaling and polishing?

Plaque is the soft film that forms on teeth every day. Left alone it hardens into calculus, also called tartar, which is rough, porous and bonded to the tooth. No toothbrush removes calculus. Scaling is the procedure that does, with an ultrasonic tip and hand instruments, and polishing smooths the surface afterwards so plaque takes longer to stick again.

The reason it matters is the gum, not the appearance. Calculus sitting at the gum line keeps the tissue inflamed, which is gingivitis. Left long enough the inflammation moves deeper, the bone around the tooth is lost, and that is periodontitis, the most common reason adults in Pakistan lose teeth that were never decayed. Calculus

Hardened, mineralised plaque bonded to enamel and root surface. It cannot be brushed off, and its rough surface collects more plaque, which is why the cycle accelerates.

Part 3

Stain

Tea, coffee, paan and smoking stain the surface. Polishing lifts surface stain, though it does not change the natural colour of the tooth, which is what whitening is for.

What does scaling cost in Islamabad?

A routine scale and polish is one of the least expensive things done in a dental clinic, and deep scaling is priced by quadrant because it takes anaesthetic and considerably more time. You are told which one you need after the gums are measured, not before.

A word about very cheap scaling offers. A three thousand rupee clean advertised as a deal is usually ten minutes with an ultrasonic tip above the gum line. If your gums bleed and the pockets are deep, that appointment leaves the disease exactly where it was. Ask whether the gums will be measured, and ask what the plan is if they are not healthy.

Does scaling damage or loosen teeth?

No, and this is the most persistent dental myth in Pakistan. Ultrasonic scaling removes calculus, not tooth. Enamel is harder than the deposits being removed, and the instrument works by vibration rather than by cutting.

The feeling that teeth are loose afterwards comes from what was holding them together. Heavy calculus splints teeth that gum disease has already loosened; once it is removed you feel the movement that was there all along. The looseness is the disease showing itself, not the scaling causing it.

Sensitivity for a few days is normal, particularly where the gum has receded and root surface is exposed. It settles, and a desensitising toothpaste helps. The NHS guidance on gum disease says the same about scaling and gum disease, and is worth reading if you have been told otherwise.

What happens at a scaling appointment.

A routine appointment takes about thirty to forty minutes. Deep scaling is longer, done under anaesthetic, and usually split across two visits so each side is treated properly.

Stage 1

Gum assessment

The gums are measured around each tooth and bleeding points are recorded. This is what separates a routine clean from gum treatment, and it takes only a few minutes.

An ultrasonic tip breaks up and flushes away calculus above and just below the gum line. It feels like vibration and cold water rather than scraping.

The honest answer

Does scaling hurt?

A routine scale and polish on healthy gums is uncomfortable at worst. What hurts is inflamed gum tissue, which is exactly what the appointment is treating.

Longevity

How long does a clean last?

Calculus begins forming again within days, so the honest answer is that a clean is not a result you keep, it is a reset. What decides how long the mouth stays healthy is what you do at home between appointments.

For most people a six monthly interval is right. For smokers, people with diabetes, orthodontic patients and anyone who has had gum treatment, three to four months is the interval that actually holds, because the bacteria return on a schedule.

Cleaning between the teeth daily with the right size of interdental brush makes more difference than any toothpaste. If you wear braces or have implants, that becomes more important rather than less.

Magnetostrictive or piezo tips that break up calculus with vibration and irrigate the area at the same time, which is both faster and gentler on enamel than hand scraping alone.

Hand instrumentation and root planing

Curettes finish the surfaces the ultrasonic tip cannot reach and smooth the root so the gum can reattach, which is the part that actually reduces pocket depth.

Ask whether your gums will be measured at the appointment. A clinic that records pocket depths is treating your gums; a clinic that goes straight to polishing is treating the way your teeth look.
